## A little more about Brett Pugh.

As the founding physical therapist of the Commons Clinic rehabilitation department, Brett brings a leadership mindset and a commitment to reimagining the physical therapy experience. He delivers personalized, evidence-based care in a one-on-one setting, avoiding the high-volume models often seen in traditional clinics.

Brett’s treatment approach incorporates advanced rehabilitation techniques such as functional movement screening, blood flow restriction therapy, gait analysis, joint mobilization, and corrective exercise plans. He works closely with the broader care team to ensure each patient’s recovery is both tailored and connected, with a focus on achieving meaningful, lasting results.

> We started treatments about 3 weeks ago, my father is 89 and had a severe right shoulder fracture. Brett is an excellent professional physical therapist. > Brett Pugh, PT, DPT is a detailed and experienced physical therapist who got me back to functional health after two concurrent surgeries - a massive rotator cuff tear as well as a torn quadriceps tendon - Brett was instrumental in my regaining complete function and range of motion for both ailments! > Really bad care and bad follow up and a bad experience . Plus his exercises hurt me more even when I said I was told not to do those kinds. I was in training for the Olympics when I was young and been an athlete all my life and never had I had this bad an experience...Will never go back here ...even the Doctor that recommended him never followed up after I called and complained... Really disappointing ...

Questions for your care

## Know what *to ask.*

Choosing your care

### [What good care coordination looks like before and after treatment](https://commonsclinicreviews.com/articles/care-coordination-before-after-orthopedic-treatment/)

Good coordination turns a treatment recommendation into a plan you can follow: the right records, clear responsibilities, and someone to contact when the next step is unclear.

[Read the guide](https://commonsclinicreviews.com/articles/care-coordination-before-after-orthopedic-treatment/)

Choosing your care

### [Making a nonsurgical orthopedic treatment trial useful](https://commonsclinicreviews.com/articles/nonsurgical-treatment-trial-orthopedic-care/)

Trying nonsurgical care should come with a purpose, a practical plan, and a review point. Ask what you are testing, which improvement matters, and what would make the next decision clearer.

[Read the guide](https://commonsclinicreviews.com/articles/nonsurgical-treatment-trial-orthopedic-care/)

Knee

### [Pain around the kneecap: what should a useful care plan include?](https://commonsclinicreviews.com/articles/patellofemoral-knee-pain/)

Front-of-knee pain often needs a thoughtful movement and loading plan, not a rush to label every ache as damaged cartilage.

[Read the guide](https://commonsclinicreviews.com/articles/patellofemoral-knee-pain/)

Pain management

### [Sacroiliac joint pain: ask how the diagnosis fits your symptoms](https://commonsclinicreviews.com/articles/sacroiliac-joint-pain-evaluation/)

Pain near the back of the pelvis can overlap with lumbar and hip problems. A useful visit explains why the sacroiliac joint is suspected and how treatment will be evaluated.

[Read the guide](https://commonsclinicreviews.com/articles/sacroiliac-joint-pain-evaluation/)

Physical therapy

### [How to choose a physical therapist for your goals](https://commonsclinicreviews.com/articles/choosing-physical-therapist/)

The right physical therapist understands what you want to do, measures where you are starting, and helps you use a plan outside the clinic. Ask about experience, progress, and the practical details of visits.

[Read the guide](https://commonsclinicreviews.com/articles/choosing-physical-therapist/)

Physical therapy

### [Making a physical therapy home exercise plan you can actually use](https://commonsclinicreviews.com/articles/physical-therapy-home-exercise-plan/)

Your home program should be clear enough to use on an ordinary day. Ask what each exercise is for, how much to do, and how to adjust when the plan meets real life.

[Read the guide](https://commonsclinicreviews.com/articles/physical-therapy-home-exercise-plan/)

Physical therapy

### [Low-back physical therapy: what a useful plan should include](https://commonsclinicreviews.com/articles/physical-therapy-low-back-pain/)

Low-back physical therapy should explain what limits you, help you build a workable movement plan, and measure progress in activities you care about. Ask how the plan changes as you respond.

[Read the guide](https://commonsclinicreviews.com/articles/physical-therapy-low-back-pain/)

Physical therapy

### [When physical therapy progress stalls: questions for a useful reassessment](https://commonsclinicreviews.com/articles/physical-therapy-progress-plateau-questions/)

When progress feels slow, bring the details into the next visit. A useful reassessment looks at function, symptoms, the program you could follow, and what needs to change.

[Read the guide](https://commonsclinicreviews.com/articles/physical-therapy-progress-plateau-questions/)

Physical therapy

### [Rehabilitation after spine surgery: who should coordinate what?](https://commonsclinicreviews.com/articles/rehabilitation-after-spine-surgery/)

Rehabilitation after spine surgery needs a plan that matches the operation and the surgeon’s instructions. Clarify the handoff, the first goals, and who changes restrictions as recovery progresses.

[Read the guide](https://commonsclinicreviews.com/articles/rehabilitation-after-spine-surgery/)

Physical therapy

### [Returning to activity after a back injury: how to make the plan specific](https://commonsclinicreviews.com/articles/returning-to-activity-after-back-injury/)

Returning to activity works best when the activity is specific. Define the task, establish your current capacity, and agree on how to progress or adjust with your clinician.

[Read the guide](https://commonsclinicreviews.com/articles/returning-to-activity-after-back-injury/)

Sports medicine

### [Returning to sport after injury: questions beyond “Am I cleared?”](https://commonsclinicreviews.com/articles/return-to-sport-after-injury/)

A meaningful return plan separates participation, sport and performance—and keeps the athlete involved in each decision.

[Read the guide](https://commonsclinicreviews.com/articles/return-to-sport-after-injury/)
