Reviewer names are abbreviated. Patient reviews describe individual experiences. Review counts describe source records, not unique patients or clinical outcomes; the acquired snapshot may not include complete platform histories. Clinical education and patient accounts do not replace a consultation.

What matters to you

# Care coordination

Referrals, records, scheduling and follow-up can involve several people. Look for concrete accounts of how the team handled those steps, and confirm who will be responsible for your own next contact.

A question to bring

## Who coordinates each next step, and whom should I contact if it does not happen?

## Ask about the process behind the praise

AHRQ includes timeliness and patient-centered care within its quality framework. When a review praises helpful staff, look for the specific task: arranging a visit, obtaining a record, or clarifying a next step. Ask our team how that process works for your own appointment and what you need to provide.

## Give an unresolved task a clear owner

AHRQ’s discharge follow-up tool checks appointments, services, and problems after a transition. Use a similar practical question when something is outstanding: who can resolve this item, and how will I hear back? Identify the missing referral, instruction, or record so the next person can help with a defined task.

## Questions to bring

- Which person can confirm that my records and referral are complete?
- Who will let me know when this outstanding task is resolved?

The right records help a second-opinion visit focus on your decision. Confirm what the clinician needs, arrange a usable transfer, and bring a short treatment timeline rather than an unorganized stack.
