# A painful unstable thumb: 6 questions about ligament injury

A thumb that feels unstable after an injury needs assessment of the ligament, joint, and any associated fracture. The key question is how to protect useful pinch while the injury heals.

## 1. Which structure was injured?

A force that pushes the thumb away from the hand can injure a supporting ligament at the knuckle. Other injuries can also make the thumb painful or difficult to use. Describe the accident, the direction of force if you remember it, and whether the joint looked out of place. Ask the clinician to name the injured structure and show you the joint involved. An examination and appropriate imaging help distinguish a ligament problem from a fracture or another injury. Do not repeatedly pull the thumb sideways to compare it with the other hand.

## 2. Is the joint stable enough to heal with protection?

A mild injury and a completely disrupted ligament may require different plans. Ask what the examination shows about stability and whether the ligament is positioned in a way that affects healing. If pain makes assessment difficult, ask how uncertainty will be resolved. The answer should explain the recommendation for protection, specialist follow-up, or surgery. Avoid assuming that the ability to move the thumb means the ligament is intact. Bring any previous thumb injury into the discussion, because your usual movement and stability may already differ between sides.

## 3. What does the imaging add?

Ask whether an X-ray shows a piece of bone pulled away with the ligament or another fracture. If ultrasound or MRI is proposed, clarify the specific question it is intended to answer. More imaging is not automatically better; the useful test is the one that changes a decision. Make sure the treating clinician has access to earlier images if you were first seen elsewhere. If the diagnosis remains uncertain, leave knowing how to protect the thumb and when it will be reassessed rather than simply waiting for pain to improve.

## 4. What protection will fit the tasks I cannot avoid?

Discuss how the prescribed splint or cast affects work, bathing, dressing, and caring for others. Tell the team if your dominant hand is involved or if you need the thumb for tools, instruments, or mobility aids. Ask which movements and loads are allowed and who can adjust an uncomfortable device. A hand therapist may help with appropriate protection and task adaptations when referred. Follow the individualized instructions rather than loosening the plan because a task feels tolerable. Pain relief and readiness to resume strong pinch are not the same decision.

Consider the tasks that use pinch without much visible force, such as pulling up clothing or opening a sealed packet. Ask the team which adaptations are appropriate instead of trying each task until one hurts. If an employer or family member needs to understand the limits, request a short written description. That shared understanding can make the protection plan easier to maintain through the busiest part of the day.

## 5. If surgery is recommended, why is it preferable now?

Ask which feature of the injury makes an operation worth considering and what the reasonable alternatives are. Discuss the expected benefit for stability and pinch, the important risks, and what may remain limited afterward. Clarify whether the timing of treatment changes the options and how soon you need to decide. The discussion should include what happens if you cannot arrange the recommended recovery support. Choosing an operation is also choosing a period of protection and follow-up, so ask for those requirements before settling on a date.

## 6. How will pinch and everyday use return?

Ask which milestones will guide the transition from protection to motion and later loading. The plan should identify who supervises rehabilitation and how the surgeon and therapist will share updated restrictions. Bring a short list of the tasks you want back and rank their importance. Opening packaging, fastening buttons, and using work equipment may return at different points. Avoid testing the repair with a strong grip simply to see whether it feels better. At follow-up, report the actual tasks you can manage and any new instability, numbness, or device problem.
