# 7 checks before accepting a stem-cell or cartilage-regrowth claim

A claim about stem cells, tissue repair or cartilage regrowth deserves more than an impressive image. Ask for the exact product, the evidence for your condition and the limits of what is known.

## 1. Identify the product behind the claim

Ask what material will be collected or injected, where it comes from and how it is processed. Stem cells, platelet-rich plasma, bone-marrow concentrates and cultured cartilage products are not interchangeable. If the explanation remains a broad promise about healing, request the precise product and procedure names in writing. You should be able to take that information to another clinician. A proprietary label should not prevent a clear discussion of what enters your body or what problem it is intended to treat.

## 2. Check the exact regulatory status

The FDA warns about unapproved regenerative products marketed for orthopedic and other conditions. Ask whether the specific product is approved for the proposed use, is being studied under appropriate authorization, or is offered under another regulatory framework. A device used to prepare a product may have clearance without establishing that the resulting treatment improves your condition. Registration of a company or listing of a study is also different from product approval. Have the clinician explain the distinction in ordinary language.

## 3. Separate symptom relief from regrowth

A person may report less pain without evidence that damaged cartilage has been restored. Ask what the claim actually measures: a pain score, improved function, an imaging change, avoidance of another treatment, or new tissue. Request evidence linking that measure to outcomes patients value. A before-and-after image needs comparable imaging methods and an explanation of what changed. For advanced hip and knee arthritis, AAHKS states that biologic injections have not been shown to regenerate cartilage and does not recommend their routine use.

## 4. Make sure the research matches your diagnosis

Evidence for one tendon or a focal cartilage defect cannot be assumed to apply to widespread arthritis or persistent back pain. Ask how the study population compares with your age, severity, prior treatment and goals. The September 2026 ACR summary strongly recommends against stem-cell injections for knee and hip osteoarthritis. Its guidance is diagnosis-specific. If a clinician offers a different interpretation, ask for the direct supporting study and how it changes the decision, rather than relying on a general statement that science is moving quickly.

## 5. Ask what the comparison group received

Improvement after treatment can reflect several things, including natural symptom changes and other care received at the same time. A testimonial cannot separate those effects. Ask whether the research used a meaningful comparator, how many people completed follow-up and whether the outcome was assessed long enough to answer your question. You do not need to judge a trial alone. A clinician should be able to explain what the evidence supports, where it is uncertain and why the proposed benefit is worth considering.

## 6. Put costs and setbacks into the same plan

Request a complete estimate, including repeat procedures, imaging, laboratory work and follow-up. Ask who treats complications and what happens if the promised improvement does not occur. Understand whether standard care would be delayed and whether a prepaid package can be stopped. If a research study is involved, ask about consent, oversight, costs and contacts for participant concerns. The decision should remain voluntary and understandable. Pressure to commit immediately is a reason to pause and obtain an independent clinical explanation.

## 7. Compare with established options before deciding

Ask which diagnosis-specific rehabilitation, medication, procedure or surgical options remain reasonable. Some regulated cartilage restoration procedures have defined indications; that does not validate unrelated injections carrying a similar regenerative label. Bring the treatment proposal to a consultation and ask our team to help clarify the decision you are facing. Patient reviews may illuminate whether someone felt listened to, but they cannot establish tissue regrowth, regulatory approval or comparative effectiveness. A useful plan makes the uncertainty visible and leaves you with a choice you can explain.

## Ask what is being injected before evaluating a regeneration claim

The phrase regenerative injection can refer to very different products and procedures. Ask for the exact substance, how it is prepared, where it would be injected, and the intended diagnosis. Platelet-rich plasma, cell products, and exosome products should not be treated as interchangeable simply because an advertisement groups them together. A claim about one product does not establish the effectiveness or regulatory status of another.

FDA consumer information warns about unapproved regenerative products and misleading claims. Ask for the regulatory status of the specific product and intended use, and request a source you can check. Registration of a facility or a study listing is not the same as FDA approval. If the treatment is offered as research, ask about the study oversight, consent process, costs, and how adverse events are handled. Do not interpret the word investigational as evidence that a treatment is already proven.

Request evidence for the particular back condition and procedure being proposed. Ask what comparison was used, which outcomes were measured, and how long patients were followed. Testimonials and images described as before and after do not by themselves establish that a disc or joint regenerated. Ask what evidence would support that specific claim and what alternative explanations or uncertainties remain.

Finally, compare the proposal with reasonable established care and the option of not proceeding. Ask about total costs, follow-up responsibilities, and what happens if symptoms worsen or the intervention does not help. Bring the written offer to an independent clinician if you want another perspective. A careful decision begins with a named product, a defined clinical target, and evidence that addresses that exact combination—not an appealing umbrella term.

## Questions to bring to your consultation

Choose the questions that matter most to you. Bring them along, and ask us to explain anything that is unclear.

- What exact product and procedure are proposed?
- What is approved, and for which indication?
- Does the claim mean symptom improvement or proven tissue repair?
- Which controlled study matches my diagnosis?
- What do professional guidelines recommend for this condition?
- What are the complete costs and complication arrangements?
- Which established alternatives should I compare first?
- What exact product, target, and diagnosis are involved in this proposal?
- What is its regulatory status for this intended use and where can I verify it?
- What comparative evidence supports the claimed benefit or regeneration?
