Knee

# 6 questions after a patellar tendon rupture

The patellar tendon helps connect the kneecap to the shinbone as part of the mechanism that straightens the knee. A suspected rupture needs prompt assessment and clear protection instructions.

## 1. Ask what part of the straightening mechanism is injured

The quadriceps muscle, quadriceps tendon, kneecap and patellar tendon work together to extend the knee. Ask which structure is damaged and whether the tear is partial or complete. Inability to actively straighten the knee after an injury is an important reason for prompt evaluation. Do not repeatedly test the leg or force walking to see whether it improves. The clinician should assess the injury and explain which findings require immediate protection or further investigation.

## 2. Understand the imaging and tissue findings

Ask what X-rays or other imaging show about the tendon, attachment and kneecap position. The examination also matters; the report alone does not describe everything about function. Mention prior tendon pain, knee operations, medical conditions and medicines that may be relevant. If the injury happened with relatively little force, ask whether another health factor needs consideration. Bring earlier records when available so the team can understand whether the tendon was healthy before the event.

## 3. Discuss why repair is or is not recommended

Complete tears often lead to a surgical discussion because the ability to straighten the knee is affected; some partial tears may be managed differently. Ask why the recommendation fits your injury, what timing matters and what could make the operation more complex. The surgeon should explain whether a direct repair or another reconstruction strategy is anticipated. Discuss the risks, expected functional benefit and possibility of persistent weakness or other limitations without assuming that every rupture has the same prognosis.

## 4. Get the brace and walking instructions demonstrated

Ask how the brace should be set, when it is worn and how much weight you may place through the leg. Have a professional teach the permitted technique for transfers and walking aids. Discuss stairs, transport and bathroom access before relying on the plan at home. Do not change the brace setting because the knee feels comfortable. If the instructions are unclear or conflict with another document, contact the team for one current plan that matches your injury or repair.

Have the team address ordinary transfers as well as walking a short clinic corridor. Getting up from a low seat, entering a car and reaching the bathroom at night can be difficult while the knee is protected. If the unaffected leg, wrists or shoulders are also painful, tell the person teaching you. A walking aid is only useful when you can use it safely. Ask whether any equipment or help should be arranged before you go home, and who will check the setup if it is not working as expected.

## 5. Know how motion and strength will be rebuilt

Ask how bending, active extension and later resistance work will progress and who authorizes each change. The early goal may include protecting a repair while maintaining appropriate movement elsewhere. A calendar can outline the stages, but clinical assessment determines whether progression fits your situation. Make sure the therapist receives the operation details and restrictions. Tell the team about any fall, sudden new weakness or meaningful symptom change rather than treating it as a routine exercise setback.

## 6. Connect the longer recovery with real tasks

Discuss the strength and control needed for stairs, rising from a chair, physical work and sport. Ask which measures will guide readiness and what residual limitations are possible. Arrange follow-up that continues beyond wound healing and the end of brace use. Bring the injury records and current restrictions to our knee evaluation. Ask who will coordinate any specialized repair consultation, how your therapist will receive the instructions, and which clinician should reassess a change in function. Your own examination, progress and activity demands should drive the next decision. Include the tasks you must perform before returning to work, not only the date you hope to return.

## Questions to bring to your consultation

Choose the questions that matter most to you. Bring them along, and ask us to explain anything that is unclear.

- Which structure in the knee extension mechanism is torn?
- Is the tear partial or complete?
- What does the imaging add to the examination?
- Why is repair or nonsurgical care recommended?
- Can someone demonstrate my brace and walking instructions?
- How will motion, strength and work readiness be assessed?
