# 6 nutrition questions to discuss before orthopedic surgery

Nutrition planning should make recovery easier to manage, especially when appetite, shopping, cooking, or an existing medical condition creates a barrier. Ask for individualized advice before buying supplements or making major dietary changes.

## 1. Do I need a nutrition assessment before surgery?

Tell the team about unintended weight change, poor appetite, difficulty eating, or a restricted diet. Nutrition is one of the areas considered in the American College of Surgeons' Strong for Surgery approach. Ask whether a dietitian or another assessment would be useful before the operation.

Bring a realistic description of what you usually eat and what makes meals difficult. Include dental or swallowing problems, nausea, food access, and cultural or personal preferences. The point is to identify needs that can be addressed, not to present an idealized diet that hides the problems the team could help solve.

## 2. What should my eating plan support?

Adequate nutrition, including protein and energy, supports the body's response to surgery and tissue repair. Ask what that means for you rather than applying a universal protein target or restrictive menu. Your needs may differ because of your health history, usual intake, and the operation planned.

Ask for examples using foods you already enjoy and can obtain. If a supplement is suggested, clarify its purpose, duration, and whether it replaces or complements meals. A useful plan should explain how you will know it is working and whom to contact if it is difficult to follow.

## 3. How do my other conditions change the advice?

Tell the team if you have diabetes, kidney disease, a fluid restriction, food allergies, or another condition that affects eating. General postoperative nutrition advice may need adjustment. Ask the clinicians managing those conditions to coordinate with the surgical team where appropriate.

Avoid increasing fluids, protein, vitamins, or other products simply because an online guide describes them as helpful. Bring the exact names of supplements you already take. Ask which should continue, which require review, and whether any recommendation conflicts with an existing treatment plan. A single coordinated explanation is easier to follow than several partially overlapping lists.

## 4. How should I handle fasting and medicines around the operation?

Obtain the anesthesia team's written instructions about eating, drinking, and medicines. A general instruction to eat well before surgery does not replace the specific preparation for the procedure. Ask for clarification if your diabetes plan, medicine schedule, or usual meal timing seems inconsistent with the instructions.

Read the plan in advance and know whom to contact if something changes. If you accidentally eat or drink outside the instructions, tell the team rather than trying to compensate by withholding more food or medicine. The clinicians need accurate information to decide what is appropriate next.

## 5. Who will make meals possible after I get home?

Think through obtaining groceries, opening containers, lifting pans, reaching shelves, and cleaning up. A shoulder sling, walking aid, or spine restriction may make a familiar kitchen routine unexpectedly difficult. Consider whether you can open the packaging, carry a plate, and eat comfortably with the equipment you will use. These are useful questions for the discharge team. Ask what assistance you are likely to need and arrange it before surgery when possible.

Choose practical meals that fit your preferences and any medical advice. If another person is helping, share the plan and the foods you tolerate. Ask the team what to do if nausea, constipation, pain, or low appetite interferes with eating after discharge. Do not wait until supplies or support have run out to raise the problem.

## 6. What should prompt a call or a change in the plan?

Ask which problems with eating, drinking, swallowing, vomiting, or medicine tolerance should be reported promptly. Request guidance specific to your operation and health conditions. If a symptom is severe or accompanied by another urgent change, seek the level of assessment the team recommends rather than treating it only as a nutrition issue.

At follow-up, describe what you could actually manage, including any barriers at home. Ask whether the plan needs adjustment and whether a dietitian should help. We encourage patients to discuss these ordinary details because a recovery plan is only useful when it works in the setting where you will live and heal.
