Knee

# 7 questions before knee realignment surgery

A knee osteotomy changes alignment to redistribute load. Before choosing it, understand the part of the knee being protected, the bone-healing commitment and how it compares with other options.

[Go to your consultation questions ↓](https://commonsclinicreviews.com/articles/knee-osteotomy-questions/#consultation-questions)

## 1. Ask what the alignment measurements show

Osteotomy involves cutting and repositioning bone to change how forces pass through the knee. Ask the surgeon to show your alignment on appropriate imaging and explain which part of the joint is overloaded. The direction and location of the correction depend on the problem. A painful knee or a bowlegged appearance alone does not establish eligibility. Request a clear explanation of how symptoms, examination and imaging fit together, including any uncertainty about where the pain is coming from.

## 2. Understand the joint-preservation goal

The operation may be considered to shift load away from a damaged region in a suitable knee. Ask what improvement is realistic and which existing cartilage damage will remain. It does not replace the joint surfaces in the way a knee replacement does. If preserving your own joint is important to you, explain the activities behind that preference. The consultation should connect that goal to the condition of the knee rather than assuming that preservation is always preferable at every stage of arthritis.

## 3. Compare osteotomy with the other reasonable pathways

Ask about nonsurgical care, cartilage procedures, partial replacement and total replacement where relevant. These options address different combinations of alignment, joint damage and symptoms. Request an explanation of why one is favored and what would make the recommendation change. If another clinician proposes a different operation, compare the underlying reasoning using the same images and goals. The decision should include the expected recovery and possibility of future treatment, not only the desire to avoid a replacement today.

## 4. Clarify whether another procedure is planned

Osteotomy can be discussed alongside treatment of a cartilage defect, ligament problem or meniscus injury. Ask what each component contributes and whether they happen together or in stages. More procedures can mean different restrictions and follow-up requirements. Request the exact names and the circumstances in which the operative plan could change. If a cartilage treatment is offered, ask whether it targets a focal defect in a suitable knee or is being presented as a solution for more widespread arthritis.

## 5. Plan around bone healing

Ask how weightbearing, movement and therapy will progress while the bone heals. Find out which follow-up images or clinical assessments guide advancement. Discuss smoking or nicotine exposure, health conditions and medicines that may affect the preparation or recovery plan. The surgeon should explain risks such as delayed healing, infection and other procedure-specific complications without relying on a single risk estimate detached from your situation. Do not assume that comfortable movement means the bone is ready for unrestricted loading. Ask what would happen if healing progressed more slowly than expected, including how that would affect work leave, further imaging and the next rehabilitation stage.

## 6. Discuss hardware and later operations

Ask what fixation is used, whether it is normally left in place and when hardware-related symptoms might require evaluation. Also ask how osteotomy could affect a future knee replacement if arthritis progresses. These are planning questions, not predictions that another operation will be necessary. Understand the likely follow-up and what records you should keep. A clear discussion of the longer view helps you compare the current recovery commitment with the benefits you hope to gain.

## 7. Check whether the recovery fits your life

Bring details about physical work, driving, stairs, home support and access to therapy. Ask which activities are restricted and what milestones would allow them to resume. If the plan is impractical, that belongs in the decision before scheduling. Bring the alignment images and any earlier treatment plans to our knee consultation. Ask which clinician has experience with the specific procedure under discussion and what further evaluation would clarify your options. Leave with the next steps, anticipated recovery commitments and remaining uncertainties explained.

## Questions to bring to your consultation

Choose the questions that matter most to you. Bring them along, and ask us to explain anything that is unclear.

- Which alignment problem are we correcting?
- What part of the joint would the correction protect?
- Why osteotomy rather than nonsurgical care or replacement?
- Would another procedure be performed at the same time?
- How will bone healing guide walking and rehabilitation?
- What should I know about hardware and future replacement?
- What practical support would recovery require?
