# Could back pain need a rheumatology opinion? 6 questions to raise

Some back-pain patterns warrant evaluation for an inflammatory condition as well as a mechanical spine problem. A rheumatology referral is a question to explore with your clinician, not a diagnosis to make from a symptom checklist.

## 1. Describe the pattern across the day and night

Tell your clinician when the pain began, your age at onset, how long it has persisted, and what happens with rest and movement. Night waking, stiffness, buttock pain, and symptoms that improve with movement can be relevant to inflammatory assessment. None of these features alone proves an inflammatory condition.

Bring examples rather than broad labels. Describe what getting out of bed feels like, whether a walk changes symptoms, and how sleep is affected. If symptoms vary, explain the pattern over several days. This is more informative than deciding in advance that the pain must be mechanical or inflammatory and leaving out details that do not fit.

## 2. Mention symptoms outside the back

Psoriasis, inflammatory bowel disease, certain eye inflammation, and a family history of related conditions can matter when assessing possible spondyloarthritis. Tell the clinician about these diagnoses even if another specialist manages them. Also mention recurring joint or tendon-area problems that have seemed disconnected.

Bring the names of clinicians involved and any relevant reports. You do not need to gather every record before asking whether referral is appropriate, but a concise medical history can make coordination easier. Ask whether a symptom deserves separate assessment rather than assuming that everything is caused by the spine. If a relative has a relevant diagnosis, share its name if known rather than relying only on a family history of back pain.

## 3. Know that a single test cannot settle every case

NICE guidance cautions against ruling out spondyloarthritis on the basis of one symptom or test result. A normal plain X-ray or a negative HLA-B27 result does not automatically exclude every form of axial spondyloarthritis. The specialist considers the overall clinical picture.

If you have been reassured by an earlier test but symptoms have changed, explain the change. Ask what the test could and could not establish, whether the original question remains answered, and what would justify further evaluation. Avoid seeking repeated testing without a clear question; ask how any proposed result would affect care.

Sources: [Spondyloarthritis in over 16s: diagnosis and management — recommendations](https://www.nice.org.uk/guidance/NG65/chapter/Recommendations)

## 4. Ask what a rheumatology consultation would add

A referral should explain the concern being investigated and the information already available. Ask your current clinician why rheumatology input may be useful and whether any testing should occur beforehand. Clarify how the specialist's assessment will connect with your spine or rehabilitation plan.

Prepare a one-page history of symptoms, previous treatments, relevant diagnoses, and family history. Include what helped, what did not, and what was stopped because of side effects. Ask the referring office to share images and reports when appropriate so that the consultation can focus on interpretation rather than locating records.

## 5. Treat urgent changes as urgent, whatever the suspected cause

A painful red eye with light sensitivity or blurred vision needs urgent same-day eye assessment; do not wait for a routine rheumatology appointment. New bladder or bowel dysfunction, saddle-area numbness, or rapidly worsening weakness requires emergency assessment. These actions should not depend on first deciding whether your back pain is inflammatory.

At a routine visit, ask whom to contact for a new symptom outside the back and who coordinates care between specialties. Keep the plan accessible. Having a possible explanation under investigation should not make it harder to report a change that needs a different or more urgent assessment.

## 6. Build a plan while the diagnosis is being clarified

Ask what activity, symptom-management, or rehabilitation advice is appropriate now and what should wait for specialist review. Do not start, stop, or combine prescription treatments on the basis of a suspected diagnosis from an article. Explain how symptoms affect work, sleep, and family responsibilities so the interim plan addresses daily needs.

We encourage patients to ask for the reasoning behind a referral and a clear plan for reviewing its outcome. An attentive evaluation may involve more than one specialty. The goal is to connect the relevant findings and choose care that fits them, rather than forcing every back-pain story into one category.

## Questions to bring to your consultation

Choose the questions that matter most to you. Bring them along, and ask us to explain anything that is unclear.

- Does my pattern suggest a reason to consider inflammatory back disease?
- Which symptoms or diagnoses outside my back are relevant?
- What can my previous imaging and blood tests establish or exclude?
- What specific question would a rheumatology referral address?
- Which eye or neurological changes need urgent assessment?
- What care is appropriate while we clarify the diagnosis?
- Who will combine the findings from my different clinicians?
