# Home, clinic or video visits? 7 questions about where your rehabilitation should happen

The right setting for rehabilitation depends on what needs assessment, how safely you can travel, and what support is available. A clinic, home visit, or video appointment can serve different purposes.

## 1. What needs to happen at the visit?

Start with the task the therapist needs to assess or teach. A home visit can reveal practical barriers in the place where you live. A clinic may provide equipment or hands-on assessment that is useful for a particular problem. A video appointment can support selected assessments, education, and follow-up when appropriate. No setting is automatically best for every stage of care. Ask which parts of your plan require in-person contact and which could be handled differently. The setting should serve the clinical goal rather than be chosen only for convenience.

## 2. Can I travel safely and reliably?

Consider the whole trip: getting out of the home, into the vehicle, through the parking area, and back again after the session. Pain, fatigue, mobility restrictions, and available help can change whether an outpatient visit is practical. Tell the team if transport is likely to cause missed care. If home-based services are being considered, ask what eligibility and service arrangements apply; receiving care at home is not guaranteed by simply preferring it. A realistic travel plan makes the recommended visit schedule more likely to work.

## 3. Would seeing the home change the plan?

The narrow bathroom, entry steps, or place you sleep may be central to the task you need to relearn. Explain those details rather than saying only that you live in a house or apartment. Ask whether a home assessment would add useful information and what the therapist could address there. A visit at home can also reveal where a caregiver needs instruction. The purpose should be specific: improving a transfer, arranging safe use of an aid, or adapting an essential activity within current restrictions.

## 4. Is video appropriate for this stage of care?

Ask what the therapist can reasonably evaluate remotely and what would require an in-person visit. The answer depends on the problem, your safety, and the need for physical examination or equipment. Describe your internet access, camera setup, privacy, and ability to follow instructions. Tell the therapist if another person would need to be present. Do not attempt a difficult balance or mobility task on camera without the clinician's direction and a suitable setup. Telehealth should have the same clarity of purpose as any other appointment.

## 5. Who will coordinate changes between settings?

A transition from home care to a clinic or from in-person visits to video should transfer the relevant assessment, restrictions, and goals. Ask who sends the information and whether the receiving therapist has it. Keep the surgical or physician instructions current if they govern activity. Explain what has already been tried and what remains difficult so the next setting does not start from an incomplete history. If different teams give conflicting advice, ask them to reconcile it. A change of location should not leave you responsible for interpreting incompatible plans.

## 6. What will the setting mean for costs and support?

Confirm the service type, provider, and billing arrangement before assuming that the same benefits apply everywhere. Home health, outpatient therapy, and telehealth may be handled differently by a plan. Ask the office and insurer about authorization, network status, and your expected responsibility. Also account for transport, caregiver time, and any equipment the team says is necessary. A setting that looks cheaper per visit may create other burdens. Bringing those constraints into the discussion allows the therapist to help plan a realistic course of care.

## 7. When should the setting be reconsidered?

Agree on what would prompt a change: safer travel, a need for a different assessment, improved independence, or a task that cannot be addressed adequately in the current setting. Ask how progress will be reviewed using the same goals across visits. If video is frustrating or home visits no longer meet the need, say so. The setting is a tool within the rehabilitation plan, not a permanent label. A useful decision explains what this setting offers now and what would make another option more appropriate later.

Choose the questions that matter most to you. Bring them along, and ask us to explain anything that is unclear.

- Which parts of my assessment require in-person care?
- Can I manage the full trip to the clinic safely?
- Would a home visit answer a specific functional question?
- What can and cannot be assessed through video for my problem?
- Who transfers restrictions and progress when the setting changes?
- How do authorization and costs differ between the options?
- What would make us reconsider the setting later?
