# Knee nerve ablation: 8 questions before genicular radiofrequency treatment

Genicular nerve procedures target selected pain signals around the knee. Before considering radiofrequency treatment, clarify the diagnosis, the evidence, the role of a test block and what symptom relief would allow you to do.

## 1. Confirm the source of the knee pain

Ask what diagnosis is being treated and what has been evaluated already. Osteoarthritis, a new injury, infection and a painful replacement do not all call for the same approach. A procedure that changes pain signaling should not substitute for investigating a problem that needs a different treatment. Bring the examination findings, images, prior operation reports and treatment history. If the knee is newly hot, swollen or markedly worse, seek assessment rather than assuming that the next pain procedure is the answer.

## 2. Understand a block versus radiofrequency treatment

A genicular nerve block places medication near selected nerves and may provide temporary relief or information. Radiofrequency ablation uses energy to affect targeted nerve tissue and is a different procedure. Ask which is proposed now and how the response to one would inform consideration of the other. The nerves are around the knee; this is not the same anatomy or procedure as spinal radiofrequency treatment. Request a simple drawing that makes the intended target and purpose clear.

## 3. Discuss the current evidence honestly

The September 2026 ACR osteoarthritis summary conditionally recommends radiofrequency ablation for knee osteoarthritis, with low-certainty evidence. The AAOS knee-arthritis guideline also treats denervation evidence as limited. Those statements support a careful, individualized discussion rather than a promise of durable relief for everyone. Ask which studies fit the technique and diagnosis proposed for you, how long outcomes were followed and what remains uncertain. Evidence for osteoarthritis should not automatically be extended to unexplained pain after replacement.

## 4. Ask what the test block can and cannot predict

If a block is planned, ask what symptom or task you should assess, when to record the response and what the clinician considers informative. Follow the activity instructions rather than deliberately overloading the knee while it feels numb. Temporary improvement may help the decision but does not guarantee that later ablation will help or last a particular duration. Ask what a negative or unclear response would mean and whether another explanation for the pain should be reconsidered.

## 5. Review risks and preparation for the exact technique

Ask about bleeding, infection, nerve-related problems and other risks relevant to the procedure offered. Tell the team about blood-thinning medicines, allergies, implanted devices and other health information they request. Clarify imaging guidance, sedation, fasting and transport requirements. Do not apply preparation instructions from an unrelated injection. The clinician should explain the technique at a patient-appropriate level and provide a contact route for questions, without asking you to interpret technical needle-placement details yourself.

## 6. Set a functional goal and a review point

Choose a task that matters, such as walking a useful distance or participating in a rehabilitation plan. Ask how benefit will be assessed and what happens if relief is partial or brief. Reducing pain does not restore cartilage, correct alignment or replace the need for appropriate strength and mobility work. Discuss how the procedure would fit with the rest of treatment. A useful outcome should be recognizable in daily life rather than measured only by an immediate pain score after the appointment.

## 7. Compare alternatives and the complete cost

Ask which exercise, medication, other procedure or surgical options remain reasonable and why this option is being considered now. Request the cost of the assessment, any blocks, ablation and follow-up, with confirmation of coverage where applicable. A proposed sequence may involve more than one visit and charge. Understand whether repeating treatment would be considered and what evidence would support that decision. Cost and inconvenience belong in the comparison alongside potential relief and uncertainty.

## 8. Confirm who evaluates and performs the procedure

Our knee and pain clinicians can help assess persistent symptoms and explain an appropriate care pathway. Confirm directly whether a particular physician offers genicular radiofrequency treatment and has experience with your diagnosis. Ask which assessment should happen first and whether a different treatment addresses the cause more directly. Patient reviews can help you understand communication and follow-up, but they cannot establish effectiveness for a new indication. Leave with a diagnosis-based plan, a clear next step and an agreed reassessment.

## Questions to bring to your consultation

Choose the questions that matter most to you. Bring them along, and ask us to explain anything that is unclear.

- What diagnosis is this procedure intended to treat?
- Is the next step a block or radiofrequency ablation?
- How strong is the evidence for my situation and the proposed technique?
- What can the block response predict, and what can it not predict?
- What risks and preparation apply to me?
- Which functional improvement will we measure?
- What alternatives and total costs should I compare?
- Who performs the treatment and coordinates follow-up?
